We’re honored you’re considering Teresa’s Place Adult Daycare Center for your loved one’s care. Our center offers compassionate, personalized daytime support for adults living with dementia, Alzheimer’s, and cognitive impairments. Use this form to schedule: A virtual or in-person tour of our facility A consultation to discuss individualized care plans Enrollment appointments or general information sessions Terms & General Info: All visits must be scheduled at least 24 hours in advance. Consultations are free and typically last 30–45 minutes. Teresa’s Place does not currently offer overnight/respite stays but we will in the near future. STAY TUNED! Families are encouraged to bring any relevant care documentation for discussion. Your First Step With Teresa’s Place – Consultation & Care Planning

At Teresa’s Place Adult Daycare Center, every journey begins with a personal consultation. This is where we meet with you, your loved one, and—when appropriate—medical professionals to fully understand cognitive, medical, and social needs. You’ll tour our home-like facility, meet our compassionate team, and see how our programs are designed for seniors living with severe dementia.

During this visit, we work together to design a care plan that supports your loved one’s safety, dignity, and quality of life. We ask that families share openly about any behaviors or medical concerns so we can prepare the right environment and support.

Our Promise & Responsibility:
For the safety of all participants, Teresa’s Place reserves the right to accept or discontinue services at our discretion—especially if a client’s needs or behaviors pose a safety risk to themselves or others. While this decision is rare, it ensures that every client receives the safest and most appropriate care possible.

This initial consultation is an important step in creating a smooth transition into our program and building a relationship of trust from day one.
